Thoratec Corporation Grant Committee
The Thoratec Corporation Grant Committee funds and supports educational and charitable endeavors that promote scientific knowledge, medical advancement, effective health care delivery, and patient education in the area of mechanical circulatory support as part of the continuum of therapies for advanced heart failure, and other related topics.
The Thoratec Corporation Grant Committee reviews all funding requests for charitable contributions and educational grants with the subject matter described above. Examples of these types of programs include the following:
- Charitable fundraising
- Continuing Medical Education (CME) programs
- Grand rounds
- Symposia & Conferences
- Medical education of medical students, residents, and fellows
- Public and patient education projects
How to apply:
To apply for a grant that meets the criteria described above, please submit the following two (2) required forms:
- Grant Request Form (doc 44kb)
- Grant Agreement (doc 54kb)
In order to assure readability, please type all forms rather than handwriting.
Completed, signed forms and any additional documents indicated on the Grant Request Form may be submitted directly to the Grant Committee Administrator either by fax (925.738.0110) or by email (grantcommittee@thoratec.com).
Application Schedule:
Complete and submit the Grant Request Form according to the directions on the form. The Grant Request Form should be completed in full, accompanied by an executed Grant Agreement, and received by the Grant Committee Administrator at least four (4) weeks prior to the start of the applied for program, because the Thoratec Corporation Grant Committee meets on a monthly basis. To avoid any delays in reviewing your request, please be sure to fully complete the request form and provide all of the requested information and documents.
